Occupational Therapy Services

Occupational Therapy Functional Assessment (in home) to review client's current level of function and ability to manage personal care, housekeeping, and / or child care tasks.
Job Site Assessment (including Physical Demands Analysis) and Return to Work programs.
Attendant Care Assessment to outline personal care needs.
Ergonomic Assessment (home office or commercial setting).
Activities of Normal Living Intervention (or ANLI for MVA).
Ontario Claims Form (OCF) Completion (for MVA).
Additional Tasks including case management, doctor consultation, file review or report critique, and surveillance review and comment.
Job coaching / return to work planning for Long Term Disability Carriers.
Personal Care Allowance Assessments / Functional Assessments / Mobility Assessments (WSIB).

All services are billed hourly in 6 minute time increments. All billing reflects the true amount of time required to complete any given task.
Auto insurance fees are based on the SABS fee schedule for catastrophic and non-catastrophic injuries.
Other insurance services are based on the posted OSOT suggested OT rates and are negotiated with the funding source at the time of referral.
Kilometers are billed at .59 / kilometer.
Long distance charges are billed at .15 / minute ( Canada ) and .25 / minute (US).

Presentations (the Role of OT in the Auto Sector, The Functional Assessment Process, The Assessment and Treatment of Functional Cognition, the Impact of Injuries on Seniors).
Community Private Services.

Home safety assessments (including bathrooms, home access, or for cognitive impairment).
Device prescription (except ADP).
Home office assessment for ergonomics.
Strategic planning for returning to function following an injury.
Assistance with accessing community supports and services.
